• Chairbound/Bedbound
• Severe Shortness of Breath
• Oxygen Dependent
• Pain Restricting Movement
• Terminal Condition
• Severe Functional Limitations
• Severe Immune Suppression
• Impaired Mental Status
• Angina With Activity
• Lower Extremity Edemas Impairing Ambulation
• Pain On Movement Or Walking
• Activity Restricted Due To Pain
• Weakness Due To Chemotherapy or radiation treatments.
• medically restricted due to risk of infection post-op
• Cannot safely leave home without assistive device
• Patient unable to use stairs independently
• Second level or high home without elevators
• Severe weakness/Unsteady gait
• Confusion level precedes safe leaving of home
• Cardiac restriction due to chest pain
• Shortness of breath due to talking
• Unsafe for patient without supervision
• Cannot leave home unattended due to dementia
• Requires 24 hour care and supervision
• Requires maximum assistance for all activities
